NAMC Instructions for the 2009 Alzheimer's Memory Walk

How to Register for the NAMC Memory Walk Team

1. Go to www.georgiamemorywalk.org. You can either right click on this hyperlink and select Open Hyperlink or you can cut and paste this hyperlink into your browser.

2. Scroll down the page and click on North Fulton-Gwinnett.

3. Click on “Sign Up” in the green bar near the top of the page. Read the waiver release and then click on the “I Agree” link.

4. Click on the circle in front of “Join a Team”. Click on “Show All”. Scroll down to “North Atlanta Men’s Club” and click on “Join team” to the immediate right of our club’s name.”

5. The page will automatically expand to the “Personal Information” section. You can either register yourself or you can register both yourself and your spouse (or significant other).

a. If you have registered on-line for past Memory Walks, click “If you have participated in any Memory Walk in the past, click here to autofill this form” at the top center of the “Personal Information” page. A screen will appear for you to enter the username and password you have used in the past. (If you use the same computer as last year, your username and password will appear even if you do not remember them.) If you do not remember them, click the “Forgot Username and/or Password?” link and follow the prompts. Once you enter your username and password, your personal information will appear on the screen. Please review it and make any appropriate additions or changes such as adding your spouse or significant other. All the information you created last year such as names and email addresses of people who you contacted via the Alzheimer’s website last year are available once again this year.

b. If this is your first time registering on-line for Memory Walk, please complete the “Personal Information” section. Enter your name, or both yours and your spouse’s (or significant other’s) name. All fields marked with a red asterisk are mandatory. You will need to “create Participant HQ and password”. The next section reads: “If you would like a personalized link for your web page, you must create it now. NOTE: This link cannot be edited later.” This is optional, but it is the only time you can create a personalized link.

6. There is no fee required to participate in the Memory Walk. There is no requirement for NAMC members to raise any money. A message will appear stating that participants must raise $150 to get the memory walk shirt. Please ignore this message.

7. If you choose to make a personal donation at this time, you can input the amount in the donation section. You will be asked for credit card details on the next page after selecting the “Continue” button. If you do not choose to make a personal donation at this time, simply scroll down and select “Continue.”

8. A default Personal Fundraising Goal of $300 amount may appear. If it does appear and your goal is different or $0, please input the amount or $0 and then select the “Continue” button.

9. Please review the additional information section regarding incentive items. NAMC recommends Team members select, “I only want a Memory Walk T-Shirt. Please donate unused incentive item funds back to the cause.” Of course, you need not decline the other incentive items.

10. For NAMC members who use the internet to register but who do not want to use the internet to

fundraise or have donors who do not wish to use the internet to make donations, attached is a form you can give to donors. Before sending the form, please fill in the field "participant's name" with your own name, so the Alzheimer's Association will know to record the contribution under your name. I also attach a sample letter you may wish to send to donors with the attached donor form.

11. For NAMC members unable to register using the internet please complete and send the attached Memory Walk Registration Form. As noted before, disregard the $150 minimum to receive a tee shirt. However please check off the incentive items option you choose. If you want to donate and/or fundraise please follow the instructions in #10.

Memory Walk is Georgia’s premier fundraiser to help those impacted by Alzheimer’s disease. Every 71 seconds someone is diagnosed with Alzheimer’s in the U.S. Nearly 200,000 Georgians representing every race, gender, and culture are diagnosed with Alzheimer’s disease. Unless we find a way to stop it, the magnitude of the effect of this disease is projected to triple in the next 30 years. Every step you take brings new hope for better treatments and a cure!

One of the steps I am taking is to participate in Memory Walk. I have committed to raise $________and I need your support to achieve my goal. I hope you will consider supporting my efforts on behalf of the Alzheimer’s Association. The Metro Atlanta chapter spends 79% of every dollar raised on programs and services in our community. The Alzheimer’s Association assists families with education, support groups, 24/7 helpline, and respite options. Please know your generosity is greatly appreciated.
